I authorize the use of above vehicle on the highway/streets for testing purposes if needed.

I hereby release the employees, officers from any and every claim, demand, action or right of action, of whatever kind of nature, either law or in equity, arising from or by reason on any bodily or personal injury, death or property damage, know or unknown, resulting to or resulting from the use of vehicle.

I was advised that my bike will be driven at full throttle while on the Dynamometer for tuning.

I understand and agree that payment is due at completion in cash only.

I hereby authorize and release the above Dynamometer test and/or service work to be done to the above vehicle.
